Instructions for completing the Statement of professional activities
Personal details
The statement of professional activities form is a document in which employees declare their professional activities (inside and outside the UPC).
Please note that the details provided in all sections of the form must be up to date as of the start date of your contract/appointment.
First, fill in the Personal details section.
Statement of professional activities
In these sections, fill in the details for your professional activities, both within and outside the UPC.

If you work only at the UPC:

Tick the first box: I DECLARE that I do not carry out any professional activity or hold any position in the public sector............

In the Activity 1 section, fill in the details for your professional activity at the UPC.

If you work at the UPC and also engage in another professional activity outside the institution:

Tick the second box: I REQUEST authorisation of compatibility for the indicated activities.........

Fill in the details requested in the Activity 1 and Activity 2 sections.

In the Activity 1 section, provide the details for the activity you are declaring as your main activity. In the Activity 2 section, provide the details for the activity you are declaring as your secondary activity.
Below, we provide a brief explanation of each of the sections to be filled in for Activity 1 and Activity 2.
ACTIVITY 1
Fill in the details for the activity you are declaring as your main professional activity.
Activity:
- As an employed person: If you tick this box, you must also tick the appropriate box to indicate whether you carry out the activity for a public administrative body or a private company. For a public-sector activity, indicate whether you are employed as a statutory or a contractual employee in the public administration.
- Self-employed or equivalent: Tick this box if you perform the activity on a self-employed basis.
Details of the public administrative body or company:
- Name: Indicate the name of the public administrative body or company.
- Brief description of the activity you carry out in the company: In all cases, you must indicate the type of activity you carry out in the company. This activity must be related to the field of expertise in which you will be teaching.
Full monthly remuneration (including pro rata of extraordinary payments):
- If the activity is carried out at the UPC, this section can be left blank.
- If the activity is carried out as an employed person, indicate your gross annual salary divided by 12 months.
- If the activity is carried out on a self-employed basis, indicate “variable”. (This means that you cannot determine a fixed monthly amount.
Jornada:
- Number of hours per week: Indicate the total number of hours you work. (For a self-employed activity, you can indicate “variable”.)
- Distribution of working hours: Indicate your working hours (not the total number of hours). (For a self-employed activity, you can indicate “variable”.)
ACTIVITY 2
Fill in the details for the activity you are declaring as your secondary professional activity.
If you work only at the UPC, leave this section blank.
Activitat:
- As an employed person: If you tick this box, you must also tick the appropriate box to indicate whether you carry out the activity for a public administrative body or a private company. For a public-sector activity, indicate whether you are employed as a statutory or a contractual employee in the public administration.
- Self-employed or equivalent: Tick this box if you perform the activity on a self-employed basis.
Details of the public administrative body or company:
- Name: Indicate the name of the public administrative body or company.
- Brief description of the activity you carry out in the company: In all cases, you must indicate the type of activity you carry out in the company. This activity must be related to the field of expertise in which you will be teaching.

Further information
- If you have two jobs in the public sector: you will receive the extraordinary payments and any family allowance to which you are entitled from the body indicated in section 1.
- If you have two jobs in the public sector, the one indicated in section 2 will not be considered for the purposes of determining three yearly increments, pension fund contributions or your pension entitlement.
- You must communicate any changes in the details of the activities that I carry out and have declared in this document.
- You must request authorisation of compatibility from the public administrative body for which you carry out your main activity.
Naming and saving the document
Instructions for naming and saving your statement of professional activities in the e-services portal:
- Fill in the fields of the form in PDF format using Adobe Acrobat Reader.
- Select File > Print and choose the virtual printer Microsoft Print to PDF or CutePDF Writer.
- After selecting the printer, click on the Print button. In the File name field, save the file with the name "DA.pdf".
- Select the folder or location where you want the generated file to be saved. Click on the Save button. The document will be saved in PDF format and you will have to attach it to the application you submit via the e-services portal.
If you prefer to complete the form manually, print it out using any printer available on your workstation, fill it in manually, then scan the document and save it in PDF format with the file name "DA.pdf".
Please bear in mind the following:
- Once submitted, the form and the attached document (statement of professional activities) are considered to be signed by the person concerned.
- It is important that all the details are completed in full. Any omissions could lead to erroneous interpretations that may result in your application for authorisation of compatibility being denied.
